There are three types of sleep apnea: obstructive, central, and mixed; of the three, Obstructive Sleep Apnea (OSA) is the most common. Despite the difference in the root cause of each type, in all three, people with untreated sleep apnea stop breathing repeatedly during their sleep, sometimes hundreds of times during the night and often for a minute or longer. Obstructive sleep apnea is caused by a blockage of the airway, usually when the soft tissue in the rear of the throat collapses and closes during sleep. In central sleep apnea, the airway is not blocked but the brain fails to signal the muscles to breathe. Mixed sleep apnea is a combination of the two. With each apnea event, the brain briefly awakes sleep apnea victims from sleep in order for them to resume breathing, therefore sleep is extremely fragmented and of poor quality. Sleep apnea is very common and affects more than twelve million Americans. Risk factors include being male, overweight, and over the age of forty, but sleep apnea can strike anyone at any age, even children. Yet still because of the lack of awareness by the public and healthcare professionals, the vast majority remain undiagnosed and therefore untreated, despite the fact that this serious disorder can have significant consequences. Untreated, sleep apnea can cause high blood pressure and other cardiovascular diseases, memory problems, weight gain, impotency, and headaches. Moreover, untreated sleep apnea may be responsible for job impairment and motor vehicle crashes. Fortunately, sleep apnea can be diagnosed and treated. Several treatment options exist, and research into additional options continues. How is Sleep Apnea treated? There is currently no proven drug therapy for sleep apnea. However, there are 4 basic approaches to treatment, which are not mutually exclusive: 1. Modification of circumstances which may be causing sleep apnea or making it worse. This would include weight loss, avoidance of alcohol and sedative drugs, trying to sleep only on your side and stopping smoking. It would also help to improve nasal breathing if this problem exists; sometimes this can be done with some simple medication and occasionally it requires an operation. Finally it is important to avoid sleep deprivation. 2. Use Continuous Positive Airway Pressure (CPAP) in the upper airway to support and hold the airway open. This involves wearing a close fitting mask over the nose, which is attached to a supply of continuously flowing air via a flexible plastic hose from a medical air pump that sits on the floor or bedside table. The flow of air into the mask creates a dilating pressure that is transmitted from the mask through the nose into the upper airway. This positive pressure dilates the upper airway so the breathing is not interrupted. When a sleep apnea patient no longer has obstructed breaths they are able to sleep continuously and hence their quality of sleep improves dramatically. An added benefit is that when the upper airway is dilated sufficiently there shouldnt be any further snoring. The amount of airflow and hence the amount of pressure is determined by your doctor. To date this has been the most successful and well-studied treatment for sleep apnea. This type of treatment almost always works regardless of the underlying cause of the sleep apnea. It is usually well tolerated and does not have any serious side effects. The treatment is not covered by the provincial health care plan, but is covered by some third party insurance companies 3. One of the reasons for the upper airway to become narrowed at night is because the tongue falls posteriorly, especially in the supine position. Since the tongue is attached to mandible this can be a significant problem in people with retrognathia and/or a very large tongue. A surgical operation on the back of the throat to remove redundant soft tissue in an attempt to increase the size of the upper airway can be performed. It usually involves removal of the part of the soft palate that hangs down in the back of the throat, as well as the tonsils if present, and other soft tissue if it is felt to be excessive. The operation is referred to as an uvulopalatopharyngoplasty (UPPP) and was initially described as an operation to improve heavy snoring. It is usually quite successful at decreasing the loudness of snoring but is not always successful at improving sleep apnea. There is currently no method that is widely available to predict which patients stand to benefit from this surgery. It is likely going to be less successful for patients with retrognathia. The surgery should not be considered as an option for patients with severe sleep apnea. Like dental appliances there should be some follow-up after surgery to ensure there has been a significant improvement in the severity of the sleep apnea. This surgery can be performed traditionally under general anesthesia in a hospital operating theatre. Short-term results suggest a 50% chance of improvement (defined as a 50% reduction in the AHI). Longer term studies suggest that some patients relapse and their sleep apnea is no longer controlled. Part of the reason for this may be weight gain. More recently this type of surgery is being offered to patients in an outpatient setting using local anesthesia and laser assistance. While this looks like a promising treatment for snoring there are no well-controlled, long-term studies that can demonstrate a role for it in patients suffering from significant sleep apnea. One would expect the results might be similar to the standard UPPP. Currently neither type of operation should be considered as an option for treatment unless patients are unable or unwilling to tolerate CPAP. Who should be treated? In most circumstances treatment should be reserved for those patients with particularly troublesome symptoms. What is troublesome for one patient may not be to another so that only general rules can be stated. Sometimes the patient with sleep apnea may not be the best person to decide whether their symptoms are severe or significant. Remembering the Johari Window, where the 2nd quadrant encloses the things that others know about me but I am unaware of, I realized that some of the descriptions on the ENFP personality can be considered as â€Å"things that I am not aware that I have.† It is also rather interesting that there are famous ENFP personalities according to Butt (2005), including â€Å"Meg Ryan (When Harry Met Sally), Robin Williams (Dead Poets Society, Mrs. Doubtfire), Sandra Bullock (Speed, While You Were Sleeping), Robert Downey (Heart and Souls), [and] Alicia Silverstone (Clueless)† (n.p.). These personalities have a common interest—acting. Acting specifically deals with entertaining people, which justifies the research conducted about ENFPs as being oriented with the outer world. As an ENFP type, it is safe to conclude that my interest would also be related to this field, which is unquestionably correct. The question is: What pushes me into pursuing an MBA? Honestly, it is because of the fulfillment that it brings me. I am technically new to the 16 personality types, so it is quite hard to pinpoint the motivation that I have for my decision which is also relative to my ENFP personality. Fortunately, I was able to come across one of the closest ENFP description to my personality. For ENFPs the dominant quality in their lives is their attention to the outer world of possibilities; they are excited by continuous involvement in anything new, whether it be new ideas, new people, or new activities. Though ENFPs thrive on what is possible and what is new, they also experience a deep concern for people as well. Thus, they are especially interested in possibilities for people. ENFPs are typically energetic, enthusiastic people who lead spontaneous and adaptable lives (Martin, 2008, n.p.). Apparently, one of the primary motivations here would be the involvement to something new. The enthusiasm of people with an ENFP personality guides them into taking new heights and turns along their life journey. New things excite and trigger their thirst for adventure. No matter where their personality leads them, whether interesting or not, they decide in accordance to their courage and hunger for quest. The 16 personality types are effective in discovering the reasons behind people’s decisions. The Acropolis Essays - Acropolis Of Athens, Parthenon, Erechtheion The Acropolis Elmer Boyce Architecture 110 Professor Boestler 02 November 2000 The Athenian Acropolis The Acropolis of Athens has stood as a monument of triumph to the people of Athens for centuries past. The temples within its walls were used to worship Greek gods like Athena and Poseidon. Rising over three hundred feet above the city of Athens, it can clearly be seen why it is called the Acropolis, which loosely translated means top of city. It isn't the only acropolis in Greece, but it is revered more than the others because of its almost flawless planning in where each building is placed. It took two hundred years of experimenting to get it right. Each building is placed specifically to be pleasing to the viewer's eye. From the viewer's point of view every building is seen in perspective, and at no point from the entrance is one building seen from only one facade. This is what made the Acropolis at Athens so amazing. What makes the Acropolis even more amazing is the buildings within its walls. There is the Parthenon, the Propylaia, the Erechtheion, the temple of Athena Nike, and more. After ascending nearly three hundred feet up the hill you come to what is called the Propylaia. The Propylaia is the entrance to the Acropolis and was used to prepare worshipers before entering the gates to the temples within. Construction began on the Propylaia in 437 B.C. and was completed in 432 B.C. The architect of the Propylaia was Mnesikles, and the project was anything but easy with the narrowness of space and the irregular terrain. Despite these obstacles he was successful in creating a harmonious entrance. Asymmetrically arranged buildings created the propylaia. The most important of these buildings was the Pinakotheke on the north side with contained the art gallery. The Propylaia has an outer and inner facade, both supported by six Doric columns with five doors between them, the largest door was located in the middle. Further within the Propylaia, there are Ionic columns, which helped support the roof. These columns were used rather than the Doric columns similar to the o uter columns because of space restraints. Also constructed of load bearing walls the Propylaia was entirely constructed out of white marble. These load-bearing walls were the walls of the Pinakotheke and were covered with painted panels or wall paintings. This propylaia wasn't the first on that site. The original entrance gate was smaller and was destroyed in the Persian fire in 480 B.C. After being prepared in the propylaia, you would enter a central courtyard overlooking the immense bronze statue of,the Champion, Athena Promachos. This statue was so large that the sun could be seen glimmering off the tip of its spear from out at sea. To the left you would see the Erechtheion in its white marble glory, whose site lay north of the Parthenon. This building too, like the Propylaia dealt with irregular terrain, but took it in a different perspective. Instead of leveling the land, which was too sacred to touch, they built the Erechtheion in levels to accommodate the steep change in elevation. Built sometime between 421 B.C. and 405 B.C., the Erechtheion housed shrines to several gods, local deities, and heroes. It was also the site of several sacred spots, including the mark of Poseidon's trident spear, the graves of the legendary Erechtheus and Kekrops, but most importantly it housed the temple of Athena Polias, protectress of the city and goddess of the hearth. Each level of the Erechtheion had a specific purpose. To the east, from higher terrain is a six-column Ionic porch that housed the ancient wooden image of Athena. At the north is another Ionic porch that leads to the chamber of Erechtheus. The sacred olive tre e of Athena is located in an open courtyard in the west of the Erechtheion. And finally to the south is the resting place of the legendary King Kekrops. The Porch of the Caryatids covers this gravesite. Diabetes Research Essay Diabetes is often referred to by doctors as diabetes mellitus. This describes a group of diseases in which the person has high blood glucose (blood sugar). This could be because either insulin production is inadequate or the body’s cells do not respond properly to insulin. There are 3 types of diabetes: 1)Type 1 Diabetes: The body does not produce insulin. People usually develop type 1 diabetes in early adulthood or teenage years. Approximately 10% of all diabetes cases are type 1, it is nowhere near as common as type 2. Patients with type 1 diabetes will need to take insulin injections for the rest of their life and carry out regular blood tests and a special diet to ensure proper blood-glucose levels. 2)Type 2 Diabetes: The body does not produce enough insulin for proper function, or the cells in the body do not react to insulin. Some people may be able to control their type 2 diabetes symptoms by losing weight, a healthy diet, and doing lots of exercise. They would also have to monitor their blood-glucose levels. Although the patients will most likely end up having to take insulin in tablet form because type 2 diabetes is typically a progressive disease. 3)Gestational Diabetes: This type affects woman during pregnancy, some woman have very high levels of glucose in their bodies and are unable to produce enough insulin. The majority of gestational diabetes patients can control their diabetes with exercise and diet. Undiagnosed or uncontrolled gestational diabetes can raise the risk of complications during childbirth. History For 2,000 years diabetes has been recognized as a devastating and deadly disease. In the 17th century a London physician, Dr. Thomas Willis,  determined whether his patients had diabetes or not by sampling their urine. If it had a sweet taste he would diagnose them with diabetes mellitus- honeyed diabetes. In the early 20th century, diabetologists such as Dr. Frederick Allen prescribed low calorie diets-as little as 450 calories per day for his patients. His diet prolonged the life of people with diabetes but kept them weak and suffering from near starvation. In his book, The Discovery of Insulin, Michael Bliss describes the painful wasting death of many people with diabetes before insulin: Food and drink no longer mattered, often could not be taken. A restless drowsiness shaded into semi-consciousness. As the lungs heaved desperately to expel carbonic acid (as carbon dioxide), the dying diabetic took huge gasps of air to try to increase his capacity. Air hunger the doctors called it, and the whole process was sometimes described as internal suffocation. The gasping and sighing and sweet smell lingered on as the unconsciousness became a deep diabetic coma. At that point the family could make its arrangements with the undertaker, for within a few hours death would end the suffering. Then in 1921 something truly miraculous occurred in Ontario, Canada. A young surgeon Frederick Banting, and his assistant Charles Best, kept a severely diabetic dog alive for 70 days by injecting it with a murky concoction of canine pancreas extract. Since insulins discovery, medical breakthroughs continued to prolong and ease the lives of people with diabetes. In 1935 Roger Hinsworth discovered there were two types of diabetes: insulin sensitive (type I) and insulin insensitive (type II). By differentiating between the two types of diabetes, Hinsworth helped open up new avenues of treatment. In the 1950s, oral medications-sulfonylureas were developed for people with type II. These drugs stimulate the pancreas to produce more insulin, helping people with type II diabetes keep tighter control over their blood sugars. In the 1960s urine strips were developed, and Becton-Dickinson introduced the single use syringe in 1961. This greatly reduced the amount of pain from injections as well as the time-consuming ritual of boiling needles and glass syringes. Chemistry behind diabetes Diabetes is characterized by elevated levels of glucose in the body. The management of insulin therapy in relation to carbohydrates can be hard to deal with for most diabetics. Carbohydrates lead to increased blood sugar. Once digested, carbohydrates break down into glucose molecules, which are then converted into ATP in the cells with insulin. Too little insulin causes an excess of unconverted glucose, and too much insulin causes low blood sugar. Wonderment and Awe: the Way of the Kami When watching the fantastic anime (animation) of Japanese director Hayao Miyazaki, it soon becomes apparent that he has infused his richly detailed worlds with an animistic world-view that references ancient Japanese beliefs, practices and myths. His films describe an intriguing mixture of earthy spirituality particularly drawn from the Shinto tradition. Shinto is less a religion than a way of life – a pantheistic and animistic faith that believes that every object possesses a spirit, and encourages nature worship, folk beliefs, ancient deities and rituals. It has no dogma or moral doctrine, except for four general tenets: worshipping and honouring the kami; love of nature; tradition and the family; and cleanliness (Picken 1994:9-10). For the scope of this article, I will be looking at how respect for the kami and nature inform two of Miyazaki’s films Princess Mononoke (Mononokehime 1997) and Spirited Away (Sen to Chihiro no Kamikakushi 2001). The key to Miyazaki’s work lies in his knack of transformation and transfusion. He transforms and reinvigorates the tenets of Shinto and also elements of Japanese myth such as dragons and gods. His films do not rework specific stories – rather he creates a hybrid Japanese ‘modern myth’ that is accessible (in different ways) to post-industrialised audiences all over the world. According to this, children develop their positive or negative worldview. Also person who has good characteristics will study, work hard and develop his experience it will influence his development in life. In conclusion, I believe that the life experience has a great influence on our character and the man is never too old to learn. Is your memory changing with age? People who believe they have a poor memory are usually no worse at remembering than whose who believe they have a good memory. One theory for this is that people are influenced by their general beliefs about how memory changes with age. If you believe your memory will get worse as you grow older, you will pay more attention to memory failures and each bout of forgetfulness will reinforce your belief that your memory is getting worse. Common everyday memory failures tend to be judged more harshly when these failures belong to an older person. A large scale study over a ten year period found that cognitive decline is not a normal part of aging for most elderly people. 70% of the adults in the study showed no decline in memory.
